What is a Recurrent Fissure?
Anal fissures are superficial tears that occur in the anal area. While most heal in a short time, some may become chronic and recur frequently. This condition is referred to as a “recurrent fissure.” Recurrence is usually caused by inadequate treatment, poor habits, or underlying muscle dysfunction. Especially in patients previously treated with creams or Botox, symptoms often return before full healing is achieved.
Why Do Fissures Keep Coming Back?

Treatment Strategies for Recurrent Fissures

Every recurrence should be assessed as a new case. This involves physical examination, rectoscopy, and if necessary, endoanal ultrasonography to evaluate internal muscle structure. Spasms, sphincter injury, or secondary infections are identified. - Continued Medical Treatment
In mild recurrences, treatment with creams (nitrate-based or calcium channel blockers) and warm sitz baths may be effective. However, this process requires a patient approach of 6–8 weeks. Inconsistent treatment often results in failure. - Botox Therapy (Sphincter Relaxation)
In cases where muscles don’t relax sufficiently, Botox injections into the internal sphincter can help by reducing tension for 3–6 months. During this period, the fissure can heal, and the risk of recurrence is reduced. However, in some patients, Botox alone may be insufficient. - Surgical Intervention (Lateral Internal Sphincterotomy)
For chronic, recurrent fissures that do not respond to Botox, surgery may be required. Lateral internal sphincterotomy involves partially cutting the internal muscle to relax it. This method is among the most effective, with a success rate over 90%. When performed by experts, the risk of incontinence is minimal. - Supportive Therapies
To accelerate healing and reduce recurrence risk, methods such as PRP, mesotherapy, or laser-assisted tissue repair may be used. These treatments enhance healing, reduce pain, and increase tissue elasticity. - Lifestyle Modifications
